# Referral Management Software

# Referral Management Software for Both Sides of the Handoff

A referral is easy to send. Knowing whether the patient got
scheduled is the hard part. LeadingReach helps the practices
sending referrals and the ones receiving them see where each
one stands, and follow through.

What is referral management?

## Sending the referral is
the easy part

Referral management is how a practice sends, receives, and
tracks referrals until the patient is scheduled and the loop is
closed. Some call it patient referral management. Either way, it
covers two jobs: the practice that sends needs to know what
happened, and the practice that receives needs to turn the
referral into an appointment.

Most teams do both jobs with faxes, spreadsheets, shared
inboxes, and phone calls. It works until volume grows.

Why referrals stall

## Different seats, same blind spot

Your EHR is built to document care, not to follow a referral from
“sent” to “scheduled.” So the work ends up scattered, and each
side is left guessing. The cost shows up once you measure it.

A patient leaves with a referral, and the practice that sent it has no easy
way to confirm they were seen.

## 38%

**Of referrals never close**
**the loop**

## 50%

**Of referring providers never hear**
**what happened to the patient they sent.**

![](https://leadingreach.com/wp-content/uploads/2026/10/receivers-lose-track-purple-1.svg)### Receivers lose track

Referrals arrive through different channels, ownership is unclear, and it’s hard
to say how many turned into appointments.

When that gap goes unmanaged, patients wait longer, some never get scheduled, and volume drifts out of network.

What good looks like

## Referral management that
works for both sides

![](https://leadingreach.com/wp-content/uploads/2026/10/practice-sending-mint-1.svg)#### For the practice sending

You know where a referral stands without picking up the phone.
The other office gets what it needs, you hear back, and your
records show completed care, not just sent referrals.

Referrals land in a place your team works from, whether they
arrive digitally, through your website, or as a fax your team
drops in. Someone owns each one, you can see what’s pending,
scheduled, or stalled, and you can measure how many referrals
become appointments.

LeadingReach Core

## Referral operations,
built around how
you work.

Core comes with a dedicated Account Manager who helps set
up your statuses and workflows.

Core is the LeadingReach plan for teams managing real referral
volume. It builds on our Free and Basic plans, adding to the
Network, your profile, and HIPAA-secure messaging with tools
to run a referral pipeline:

- **Pipeline management with custom statuses** built around
    your workflow
- **A real-time dashboard** that shows volume, stalled
    referrals, and your referral-to-appointment ratio
- **Digital intake** that routes referrals from your website into
    your pipeline
- **Automated patient reminders** by text and email
- **Task management and team messaging** so work stays
    with an owner
- **OutReach CRM** for physician liaison visits and
    relationships

Drag and Drop Document Upload

## Drop in the fax.
Skip the retyping.

Referrals still arrive by fax, and Core meets your team where
they are. Drag up to 15 faxed referrals into LeadingReach at a
time. The system reads each one, pulls out the patient
information, and pre-fills the referral fields in about a minute.
Your team reviews, verifies, and adds what’s missing.

Faxed referrals then live in your pipeline next to the ones that
arrive digitally, so your team works from one place no matter
how a referral came in.

Want faxes handled without anyone dragging and dropping?
Fax Automation, part of Intelligent Automation Services, is an
add-on that sits on top of Core.

## FAQs

What is referral management software? Referral management software, sometimes called patient referral management software, helps practices send, receive, and track referrals until the patient is scheduled and the loop is closed. It replaces faxes, spreadsheets, and phone calls with shared visibility into where each referral stands. LeadingReach does this across a Network of 125,000+ providers.

What is referral leakage? Referral leakage is what happens when a referral is sent but the patient is never scheduled with the intended provider, or never scheduled at all. It usually comes from missing visibility: the referral has no clear owner, isn’t tracked, and the sender never hears back.

What does “closing the loop” mean? A closed loop means the referring provider knows the outcome: the patient was scheduled, seen, or the referral was declined. Closing the loop matters for patient care, referral relationships, and value-based care documentation.

Why not just manage referrals in my EHR? EHRs are built for clinical documentation, not for managing a referral pipeline. Teams often end up tracking status in spreadsheets or workarounds, which don’t show volume, conversion, or where referrals get stuck. Referral management software works alongside your EHR to fill that gap.

How does Core handle faxed referrals? Drag up to 15 faxed referrals into LeadingReach at a time. Core reads each one, extracts the patient information, and pre-fills the referral in about a minute. Your team reviews and verifies, and the referral joins your pipeline alongside the ones that arrive digitally. For fax handling without the dragging, Fax Automation is available as an add-on through Intelligent Automation Services.

What is LeadingReach Core? Core is the LeadingReach plan for teams managing real referral volume. It adds custom statuses, a real-time dashboard, digital intake, automated patient reminders, and task management on top of the free Network.

Does it work with my EHR? Core runs on its own, next to your EHR. For a direct connection, Intelligent Automation Services is an add-on that sits on top of Core and reduces manual work between LeadingReach and your EHR. It’s live with athenaOne, eClinicalWorks, NextGen, with more in development.
