Weeks Into Minutes

How Pioneer Physicians Network replaced a fax-dependent referral system with real-time visibility across 18 locations—and became one of LeadingReach’s most vocal advocates.

The Challenge: Fax-based referrals, no visibility—and no reliable way to catch what fell through

Pioneer Physicians Network had built something rare in Northeast Ohio: an independent primary care network with 109 providers across 18 locations, connected to 270 specialist organizations and 1,900 specialty providers. The clinical reputation was strong. The referral infrastructure was not.

For years, referrals ran almost entirely on fax. When those faxes failed—system outages, undelivered documents, attachments running hundreds of pages—the team had no way to know. Referrals sat unreviewed for weeks. Back-and-forth calls to specialists piled up. And staff had no reliable way to know whether a patient had been seen, scheduled, or had simply disappeared.

“The number of faxes that weren’t received or failed to deliver because of a system outage, knowing we couldn’t pinpoint the status of those referrals, was challenging and inconvenient. We knew our current method wasn’t sustainable.”

The case for change was clear. But with a network this size, so was the risk. Before committing, Pioneer needed confidence that a new workflow wouldn’t come at the expense of patient care.

“We were concerned that having a separate login for our referral management efforts would impact the ways our patients were being seen. At the end of the day, our mission is to provide top-tier care, and we didn’t know if adopting and implementing a new workflow would hinder what we set out to do”

The Solution: Real-time referral management built for a 270-organization network

Pioneer implemented LeadingReach Core and LeadingReach Analytics to replace their fax workflow with one built on real-time visibility, standardized processes, and data-backed specialist relationships.

The shift was immediate. Custom referral statuses gave the team a single view of every referral at every stage—and eliminated the back-and-forth calls to specialists that had consumed so much time.

“The way we can see all our custom referral statuses in a single place has been instrumental in our change management journey. Not only that, but the fact that we no longer have to make phone calls to our specialists is a gamechanger in terms of operational efficiency gains”

With a network spanning 270 specialist organizations, Pioneer also needed a way to manage specialist performance at scale. The Specialty Provider Scorecard became central to how the team monitors and strengthens their Tier 1 relationships—catching gaps before they affect patient care. Amber runs scorecards once a month for several practices to gauge how their specialists are performing with the referrals Pioneer sends them.

“As soon as it’s identified that referrals aren’t getting worked, we’re able to have healthy communication and address them in a timely and proactive manner”

For a team accustomed to chasing faxes and waiting weeks for updates, the data layer changed everything.

“Having full transparency into our referral data provided the clarity we didn’t know we were missing out on.”

The Results: From weeks of uncertainty to minutes, and a 100% referral-to-response rate

Two years into their LeadingReach implementation, Pioneer’s outcomes reflect what visibility and standardization can do at scale.

“Now that we can see the status of those referrals in minutes, not weeks or months like it sometimes used to take, has been such a breath of fresh air for us.”

Akron Digestive Disease Consultants, one of Pioneer’s highest-volume specialty groups receiving hundreds of referrals each month, is a direct beneficiary of the streamlined workflows.

Pioneer went from a fax-dependent operation with no referral visibility to a proactive network that monitors specialist performance, catches gaps early, and keeps patients moving through their care journey. And they haven’t kept the results to themselves.

“I hope everybody in healthcare moves away from faxing. Using LeadingReach has allowed us to significantly improve our processes from what we were doing before. We are constantly emailing our specialists recommending they join LeadingReach because of the value we see daily.”
